About us
Nursing care built around the person receiving it
At Home Healthcare LLC provides private duty nursing for children, adults and seniors with ongoing medical needs — delivered where people would rather be, at home.
Our mission
To provide skilled, individualized nursing care in the home — so that children, adults and seniors with ongoing medical needs can live where they are most themselves, supported by nurses who treat them and their families with respect.
Our vision
A Minnesota where needing complex nursing care does not mean leaving home, and where families are partners in care rather than bystanders to it.
What we stand for
Our values
These are commitments about how we intend to work. They are the standard we want to be held to.
Individualized care planning
Every plan of care starts from one person's needs, routine and goals — not from a template with a name typed into it.
Family-centered communication
Families know things that charts do not. We ask, we listen, and we document what we are told so it reaches the rest of the care team.
Compassion and dignity
Care happens in someone's home, around their people and their routines. We behave like guests who are also professionals.
Clinical coordination
We work with providers, case managers, therapists and suppliers so that the plan at home matches the plan everywhere else.
Honest conversation
We would rather tell a family what we can realistically staff than win a referral we cannot cover well.
Practicing within scope
Our nurses work within Minnesota's nurse practice act and the written plan of care, and we say plainly when a need falls outside what we provide.
How we think about the work
Our care philosophy
Nursing at home is a different discipline from nursing in a facility. There is no call light, no colleague down the corridor, and no shift handover in a quiet office. The nurse is the clinician in the room, and the room belongs to someone else.
That shapes how we think about the work. We look for nurses with sound independent judgment and the humility to learn a family's baseline before assuming they know it. We expect documentation that would let a stranger pick up the thread. And we expect our nurses to tell us early when something is not right, rather than managing it quietly.
We also think honesty is a clinical quality. Home nursing across Minnesota is constrained by how many nurses are available, and families deserve a straight answer about coverage before they rearrange their lives around a promise.

What we commit to

To families
Clear communication, documented observations, and a direct answer when you ask how things are going — including when the answer is difficult.
To referring clinicians
A realistic response about staffing feasibility before a discharge date depends on it, and coordination that does not leave you chasing us.
To our nurses
Orientation to each case before a solo shift, accessible clinical support, and schedules built from what was actually agreed.

Licensing
Minnesota Department of Health-licensed Comprehensive Home Care Provider.
A home care license describes what an agency is permitted to provide; it is not a statement about outcomes, and it does not by itself mean a particular payer, program or county is covered. You can verify any Minnesota provider’s license with the Minnesota Department of Health.
