For healthcare professionals

Referring a patient for private duty nursing

We would rather give you an honest answer about coverage early than accept a referral we cannot staff well. Here is how the process works.

Who refers to us

Referral sources we work with

  • Hospital discharge planners

Check staffing feasibility before a discharge date is committed, and get a straight answer about what we can cover.

  • Physicians & advanced practice providers

Discuss whether extended-hour nursing fits a patient's needs, and what the order and authorization path usually involves.

  • Case managers & care coordinators

Talk through nursing level, shift patterns and coverage in a specific area before building it into a plan.

  • Social workers & family referral sources

Start the conversation on behalf of a family, or point them to us directly if they would rather call themselves.

The process

From first contact to start of care

Timelines vary with the payer and the complexity of the case. What stays constant is that clinical information moves through a secure channel, not through this website.

  1. 1. Start the conversation

Call us, or email the referrals desk. Tell us the population, area, timeframe and general nursing level — nothing that identifies the patient.

  2. 2. We assess feasibility

We look at whether we have nurses near that location with the competencies the care requires, and we tell you what we can realistically staff.

  3. 3. Secure clinical exchange

If it looks workable, we confirm a secure channel with you and the clinical documentation moves through that — orders, assessments and records.

  4. 4. Authorization and plan of care

Extended-hour nursing is ordered by a provider and reviewed by the payer. We coordinate with you and the family through that process.

  5. 5. Onboarding and start of care

Nurses are oriented to the case before a solo shift, and we confirm the communication route for ongoing coordination.

Privacy

Two separate channels, on purpose

General inquiries and clinical information are handled differently, because they carry different risk.

We do not accept protected health information through this website, and no secure clinical intake portal is published here. Any agreement needed to exchange PHI — including a business associate agreement where applicable — is handled directly with your organization before clinical information changes hands.

Not sure whether we can take a referral?

Ask us before the discharge date depends on it. We will tell you plainly what we can staff.
