Hospital & clinical referrals

Call or email with the shape of the referral — population, area, timeframe and nursing level — and we will tell you what we can realistically staff.

Referral contacts

How to reach the referral team

There is no referral form on this website. Referrals start with a conversation, so we can give you an honest answer about staffing before a discharge date depends on it.

Telephone
(612) 504-2029Fastest route. Ask for the referral team.
Fax
(612) 800-7990For documents the agency has confirmed may be faxed.
Referral email
referrals@athhc.comGeneral referral inquiries, without patient identifiers.
Intake (secondary)
intake@athhc.comIf the referral team is not the right desk.

Clinical records and protected health information

Send clinical records, orders, assessments and anything carrying a patient identifier only through a channel the agency has approved for that purpose. Please call before sending clinical documentation so we can confirm the right route for your organization.

We do not describe ordinary email or fax as HIPAA-compliant. The approved secure channel, and any agreement required to use it, is confirmed with your organization directly.

No referral form, by design

A web form would be an ordinary, unsecured channel. Rather than invite patient details into one, this site publishes the phone, fax and mailbox that reach the referral team directly.

How the referral process works

What helps us respond quickly

  • The population and general nursing level required
  • The city or county where care would be provided
  • The shift pattern being considered
  • Whether a discharge date is already set
  • Which payer is involved, in general terms
